How to Become a New Patient
A Referral Is Required to Begin Care
To be seen as a new patient at Compassionate Pain Care & Wellness Center of Connecticut, a referral from your current healthcare provider is required. This ensures Jody has the clinical details necessary to deliver safe, thorough, and personalized care from your very first visit.
Please ask your provider to fax the following to 1-800-994-9501:
Recent office notes related to your pain condition
Relevant imaging such as X-ray, MRI, CT, or ultrasound
Current medication list
Any pertinent lab work
Once your referral and documents have been received and reviewed, the practice will contact you to schedule your appointment.

What to Expect and How to Prepare
To help Jody make the most of your time together, please bring:
A list of your current medications including dosages
Any recent imaging reports if not already faxed
Your insurance card and a photo ID
A list of treatments you have tried and whether they helped
Any questions or concerns you want to discuss
Plan to arrive 10 to 15 minutes early to complete any remaining paperwork. Wear comfortable clothing, bring any mobility aids or braces you use regularly, and do not worry about minimizing your symptoms. Come as you are. Everything you share here matters and will be taken seriously.
Following your evaluation, Jody will build a personalized treatment plan around your specific needs and goals.
You will always leave your appointment knowing exactly what comes next. No confusion, no unanswered questions, no feeling like you were just another chart.
